Milwaukee County Falls Prevention Coalition is an interprofessional group of community members, health systems, and government agencies that aims to increase collaboration and establish a framework to promote fall screening and prevention in Milwaukee County.
Milwaukee County Falls Prevention Coalition is an interprofessional group of experts and advocates collaborating to prevent falls in the adult community by improving access to programs, interventions, and resources to reduce the risk of falls.
Mission
Access: Increase access to fall screening and fall prevention interventions in all communities in Milwaukee County.
Awareness: Create broad awareness of fall prevention within the community through branding, community education & communication plan.
Collaboration and Member Engagement: Increase collaboration through improved handoffs amongst community partners and strengthening membership engagement within the coalition.
Data and Metrics: Capture data to monitor the success of fall prevention initiatives and to identify patterns and prevent falls within the community.
Education: Educate providers on evidence-based screening & interventions & provide access to resources to support the community.
